The short answer
So which one you can use depends on the DDD program you are enrolled in. Your support coordinator can tell you which program that is.
- Individual Supports: a Community Care Program (CCP) service only.
- Community-Based Supports: a Supports Program service only.
- Respite: available in both programs.
What Individual Supports look like
At Tinash, Individual Supports are one-to-one services in the person's home. A trained Direct Support Professional helps with daily living, such as self-care, meals and household routines, and builds the skills to do more independently. Hours and goals are set in the service plan.
What Community-Based Supports look like
Community-Based Supports pair the participant with a trained staff member for one-to-one time out in the community: errands, shopping, classes, worship, recreation, and practicing travel, money and social skills. Goals are tracked against the service plan.
The two DDD programs
- Supports Program: for adults 21 and older found eligible by DDD and enrolled in Medicaid. There is no waiting list to enroll.
- Community Care Program: has the same basic rules plus an institutional level of care, Medicaid income and resource rules, and a waiting list unless DDD finds the person at imminent risk of homelessness.
Services available in both programs
Besides respite, DDD lists many services in both programs, including support coordination, day habilitation, supported employment, career planning, behavioral supports, community inclusion services, assistive technology, transportation and others. Check DDD's programs page for the full list.
